If you have more than one receipt, please provide the most recent retailer and purchase date, along with the combined purchase total from all receipts.

For each eligible item on your receipt, please enter the following information.

  • The 12-digit UPC code from the back or bottom of your product package.
  • The price paid per item after coupons and discounts have been applied.
  • The total quantity of the item purchased.

To assist us in verifying your purchase, please circle each eligible line on the receipt along with the total at the bottom. Items that cannot be clearly identified on the receipt will be disregarded for the purpose of qualifying your rebate.
